- ItemAdaptation en lentille sclérale par AS-OCT(2025-06-01) BELGACEMI Fatma Ibtihal; BOUKHATEM Ahlem SafiaIntroduction : Scleral lenses represent an essential therapeutic solution for the management of complex corneal pathologies such as keratoconus, corneal grafts, and severe dry eye disease. However, traditional methods for fitting these lenses have limitations in terms of precision and reproducibility. The aim of this thesis is to evaluate the contribution of anterior segment optical coherence tomography (AS-OCT) in optimizing the scleral lens fitting process. The study seeks to demonstrate that this technology allows for greater accuracy, significant time savings, and improved patient comfort compared to conventional techniques. Patients and Methods : The study included 12 patients, totaling 20 eyes, all with various indications requiring scleral lens adaptation. Inclusion criteria required a complete clinical file and a justified medical indication. Two types of scleral lenses were used: 15 mm diameter Scleraflex SwissLens and 16.5 mm diameter ICD HD Toric lenses. Results : From a functional standpoint, a significant improvement in visual acuity was observed. The most notable clinical cases involved successful adaptation in advanced keratoconus with prolonged stability, as well as post-graft patients showing excellent tolerance and satisfactory functional vision. Conclusion : This study demonstrates that AS-OCT is a major advancement in the field of scleral lens fitting. The main benefits identified include increased accuracy in clearance and alignment measurements, enhanced efficiency with fewer trial lenses required, and improved safety by preventing potential complications.
